Fall 2009 Nomination Form

Staff members are asked to consider students with whom they work or have worked. Nominees should be undergraduate or graduate students who must be graduating from SMU in December 2009 and have contributed to your office or area in a significant way, worked unselfishly and "above the call of duty" but have not yet been recognized. Please note, this award is not just a "Thank You" for ordinary service, but a recognition of exemplary and outstanding service.

We ask that you nominate ONE student for this award. Please note that this will be enforced in order to honor the significance of this award. In so nominating, your presence is required at the reception, as you will be presenting the award to your nominated student when introduced by Dr. White. The reception and presentation will be held in the Commons of Hughes-Trigg Student Center on FRIDAY, DECEMBER 18, 2009 at 3:00 PM.
