Worker's Compensation

YOU hAVE 48 hOURS TO rEPORT AN oN-THE-jOB iNJURY!

Supervisors must:

• Report the injury to the Office of Risk Management by the end of the shift                                   

• Conduct the initial accident investigation

• Complete the First Report of Accident/Incident Form and return it to Risk Management within 48 hours

• Contact the Office of Risk Management for the nearest Worker's Compensation approved facility
