Print and complete one medical waiver form per camper. Medical waivers must be submitted prior to the first day of camp and may be mailed to the address listed below or faxed to 214-768-1422.

Written cancellation will be required for any refund. An email to
firstname.lastname@example.org is acceptable. A full refund will be granted, less a $75 administrative fee, if the cancellation is received in writing by April
30th, 2013. No refunds will be granted after April 30th, 2013.
