Music Educators Workshops:

Registration

Tuition for all courses may be paid by check, money order, MasterCard/Visa, or purchase order. If using a purchase order for payment, process the P.O. using a copy of the following  registration form as an invoice; submit the P.O. with the completed registration form to SMU's Music Educators office. Certificates of attendance are provided to all individuals whose registration fees have been paid in full and who attend all class meetings.

Classes are subject to cancellation due to insufficient enrollment.

Mail: Complete the form below, print it, and then mail it with a check, credit card information, or copy of a P.O. to SMU Meadows Community Education, Music Educators Workshops, PO Box 750356, Dallas, TX 75275-0356. You may wish to make a copy of the form to keep for yourself. You will receive a confirmation of your reservation by e-mail or postal mail.
