If you are an international* student with an F-1 Visa and are enrolled in 1 or more credit hours, select the scenario below that applies to you:

  • If you are provided comparable insurance through your embassy, then you must download and complete the INTERNATIONAL WAIVER (pdf) and supply documentation with the waiver to Krista Pruitt at the Health Center. (Fax: 214-768-2151 Email: studenthealthinsurance@smu.edu)

  • If your spouse or parent works for a U.S. employer and is covered under the U.S. employer's insurance policy and you are a dependant or spouse on this U.S. plan, then you must complete an INTERNATIONAL WAIVER (pdf) and supply documentation with the waiver to Krista Pruitt at the Health Center.   • If you are not provided comparable insurance through your embassy, then you must ENROLL in the Student Health Plan. Go to: https://access.smu.edu (Note: The Health Insurance button in the Student Center component of Access.SMU will appear only after a student has enrolled for classes each semester.)

  • If you are not covered by your spouse's or parent's U.S. employer's insurance policy, then you must ENROLL in the Student Health Plan. Go to: https://access.smu.edu (Note: The Health Insurance button in the Student Center component of Access.SMU will appear only after a student has enrolled for classes each semester.)

If you are an international student attending SMU on a J-1 or H-1 visa:

  • You are not required to maintain the University Health Insurance Plan, however, students must complete the INTERNATIONAL WAIVER (pdf). Students should select ‘Attending on J-1 Visa’ and return with documentation to the Student Health Center prior to the drop-add date each term in order to be administratively waived in Access.SMU and avoid automatic enrollment into SHIP and a semi-annual charge applied to your student account. You must download and complete the INTERNATIONAL WAIVER (pdf) and supply documentation with the waiver to Krista Pruitt at the Health Center. If you are an international student enrolled in less than 1 credit hour, then NO WAIVER is needed.

*An international student is one who is in the U.S. on a Visa.
