TETANUS: All students should have had a Tetanus booster within the last 10 years
TB (TUBERCULOSIS): All students need to complete the questions on the Health Form to see if a TB test (within the last 6 months) is required.
The above documentation may be from a Public Health Clinic, a Physician or a Transcript from a School Record.
Students who are lacking any of the required immunizations may come to the Health Center upon arrival at SMU to receive necessary immunizations. Immunization records must be complete BEFORE students will be allowed to register for classes. A HOLD will be placed on a student's access account for non-compliance.
Required for all new students who are under the age of 22. This is an update to State of Texas SB 62, which contains an update on the meningitis law, effective October 1, 2013. The meningitis immunization must have been received within the preceding 5 years, or a booster shot is required. A "new student" is defined as any student who was not enrolled in SMU classes the previous semester.
Print and then complete all pages of the Health History form. (Right-Click and select "Save as" to download the files to your local machine.)
Students may view the receipt and completion of their Health Form and Immunization Records in Access.SMU by going to: Student Self-Services > Student Center > Medical Health History.
