Flu Absence Reporting

So that SMU can report accurate data to various constituents about the number of flu cases, and also make informed decisions about University gatherings and facilities, SMU asks that everyone in the SMU community who is impacted by the flu to report their status via this form.

In addition to notifying your professor or supervisor by e-mail or phone, faculty, staff, and students should use this form to report absence from work or school for illness related to the flu only. Routine vacation days and regular medical leave (i.e. doctor appointments or other non-flu medical issues) should be reported in the normal manner via Kronos time card and monthly reporting through your department.

Please submit this form for each type of absence you have.  For example, if you are absent at first because you are caring for a family member, and then you become ill with flu-like symptoms yourself, you will submit the form two times.  You do not need to submit the form if you change your date of anticipated return to work/class – this is simply an estimation when you first submit the form; the date does not need to be updated.
