Name Correction Checklist

You will need the SMU ID of the Constituent whose information you wish to have updated or corrected.

With this form, you will be able to correct the Primary (or legal) and Preferred name types.  If you need to correct one of the other names types such as Maiden, Nickname, and Other, put the details in the box marked comments at the bottom of the form.

IMPORTANT NOTE:  Be sure to include all name parts (Prefix, First, Middle, Last, and Suffix, if applicable) when making a correction or change to a constituent's Primary or Preferred name.  If a field is left blank, the process will overwrite any existing data in that field with the blank.
