All disputes of the late payment fee MUST be submitted on this form. Please allow up to 4 weeks for reply. If you have already submitted the form, please do not resubmit. This only slows down the process. Do not submit a form to dispute the late payment fee if your payment was submitted or your financial aid was completed after your payment due date. The late payment fee under those situations will not be removed or reversed. We will begin reviewing Spring 2012 disputes on January 18th.
