Hazing Reporting Form


The purpose of this form is to notify the Office of Student Conduct & Community Standards at Southern Methodist University that a hazing incident has occurred. If this is an emergency, please call the Southern Methodist University Police Department at 214-768-3333.

The Office of Student Conduct & Community Standards will review, investigate, and/or respond to the information in this report based on requests of confidentiality, legal requirements, and University obligations. Under certain circumstances, the University may have an obligation to act upon the information received in this report even if a request of no additional action is made. Please note, failure to fully complete this form may limit the University in our response and action.
